Appointments

We know you have many choices when choosing a Dentist in Findlay, OH so we have made requesting an appointment a simple process.  If for any reason you cannot keep a scheduled appointment or will be delayed, please call us as soon as possible. We do ask for 48 hours advance notice of any appointment changes.

Insurance and Billing

We can file dental claims with most traditional insurance plans. DeClerck Family Dental does not participate in all Health Management Organizations. However, we will be happy to file your insurance claims for you as long as you are free to choose your own dentist or please see the list below for in-network contracted carriers. We also accept cash, checks, or credit cards. In addition, we offer a flexible outside financing payment plan option. Please see our Financial Coordinator for details.
